How recent does my physical exam have to be?Your physical must be within 365 days of your intended start date.
Can I submit my own physical exam documentation or do I need to use the Regis form?We prefer that you use our Regis Physical Exam form. However, if you have comparable documentation of your physical exam on another form, this will be accepted.
